
Superior Home Health and Hospice is equipped with staff that are experienced and trained to provide the most comprehensive care to our patients. Our Home Health nursing team works hand in hand with rehabilitation therapy experts, and social worker staff to develop the plans to maximize each patient’s ability to become and remain as independent and self sufficient as possible, or in the accommodation of their own home. Our Hospice team specializes in providing comfort while relieving physical, emotional, and spiritual suffering to both patient and family members during their life limiting illness.

Registered Nurses(RN) Oversee caregivers, provide medical assessments, study and update client care plans. They also work along with the licensed vocational nurses to ensure proper care is practiced for each patient.

Licensed Vocational Nurses(LVN) Deliver individualized nursing care to patients and families, and thoroughly document such services.

Aides Work with patients and assist them in their daily living activities helping them obtain a prompt recovery. Aides assist with bathing, dressing, grooming, hygiene care, and medical equipment.

Medical Social Workers (MSW) Provide counseling and access to community resources to assist with financial concerns, emotional support, as necessary. MSW assist nursing staff in providing supportive measures and services, as well as assess social and emotional factors.

Physical Therapy (PT) Helps restore individuals to optimal function and independence. PT provide traditional therapeutic techniques and modalities, as well as offer a variety of programs and services aimed at prevention, wellness and education.

Occupational Therapy (OT) Helps treat small motor functions. Rehabilitative techniques of exercise and certain devices are used to increase the patient’s ability to carry out normal day-to-day activities and to increase upper extremity function.

Speech Therapy (ST) Helps treat speech and swallowing disorders. ST presents an individually designed treatment program to maximize communication for patients.

Physician (Hospice Only) Promotes and maintains the standards of practice consistent with quality hospice care.

Chaplain (Hospice Only) Provides spiritual care to our patients, caregivers of all age groups and performs spiritual assessments, provides spiritual/pastorial support and make referrals to meet the needs of patient and family.

Homemaker (Hospice Only) Provides a safe and clean environment by providing light housekeeping, laundry services, errands and assists with meal preparation.

Volunteer (Hospice Only) Provides assistance and support without compensation to patients, families and hospice team.
